Old Hosts, Young Models: Rethinking Cancer Immune Evasion In Aging
Cancer is a disease that predominantly affects older individuals, yet much of our understanding of how tumors outsmart the body’s defenses comes from studies in younger subjects. This discrepancy raises an important question: do the mechanisms of immune evasion, the ways cancer hides from or deactivates immune cells, change as we age?
Recent research delves into this question by re-evaluating the established framework of cancer immunity in the context of an aging body. It highlights that several key aspects of our immune system, which are crucial for fighting cancer, undergo significant alterations with age. For instance, the diversity of immune cells capable of recognizing tumor cells can decrease, and the surrounding tissue environment can become more suppressive even before a tumor develops. This chronic, low-grade inflammation often seen in older individuals, termed “inflammaging,” plays a critical role in creating a less hospitable environment for immune responses.
While some age-related changes simply mean the immune system operates at a reduced capacity, others represent fundamental shifts in how it functions. Interestingly, despite these changes, older and younger patients often show similar responses to certain immunotherapies. However, this might be misleading, as chronological age can mask a complex interplay of beneficial and detrimental immune changes. The findings suggest that focusing on an individual’s “immunological age” – a measure of their immune system’s health rather than just their years – could be a more accurate way to predict treatment success.
This new perspective has significant implications for developing future cancer treatments. Instead of solely focusing on boosting immune responses, therapies might also need to address and correct the age-related changes in the body’s microenvironment to be truly effective.
